This form serves as a declaration for individuals applying for a work visa. It is specifically meant for cases where another person is submitting an online application on behalf of the work visa applicant.

The form acknowledges this arrangement and provides a structure for the applicant to sign, indicating their understanding and agreement to the statements concerning the application and any potential visa to be granted.

Additionally, the form includes sections for the applicant to choose between two options regarding the person submitting the application on their behalf: one where the person is a licensed immigration adviser or exempt from licensing and will continue to act on the applicant's behalf, and the other where the person is assisting with the online form submission without taking ongoing representation roles.

The applicant must sign Section A to demonstrate an understanding of and agreement to the statements regarding the application and any potential granted visa.

The applicant must also sign EITHER Section B or Section C to indicate the delegation of authority for someone else to submit the application online on their behalf:

• For situations where the person submitting the form is a licensed immigration adviser or falls under an exemption from licensing such as an immigration lawyer, and will continue to act on the applicant's behalf during the application processing, the applicant should sign Section B.
• If the person submitting the form is merely assisting by inputting the applicant's details into the online form and submitting it on their behalf, the applicant should sign Section C.
